Needle & Syringe Sourcing
The Basic Injection Kit Checklist
The most common supply kit for injectable testosterone/oestrogen is:
- Syringes: 1 mL syringes are commonly recommended for precise dosing of HRT medications.
- Needles: You'll need two types -- a larger gauge (18G-22G) to draw the medication from the vial, and a smaller gauge (25G-27G) for the actual injection.
- Sharps Container: A puncture-resistant container for the safe disposal of used needles. If a medical one is not available, a thick plastic laundry detergent bottle works as a temporary alternative.
- Alcohol Wipes: For sterilising the top of your medication vial and cleaning your skin before the injection.
- Bandages / Plasters: A small bandage for the injection site afterward. Not required, but recommended.
Needle Gauge Reference
| Gauge (G) | Outer Diameter (mm) | Colour (ISO) | Use |
|---|---|---|---|
| 18G | 1.27 mm | Pink | Drawing thick oil-based medication |
| 20G | 0.91 mm | Yellow | Drawing medication (good balance) |
| 21G | 0.82 mm | Green | Drawing or injecting IM |
| 22G | 0.72 mm | Black | Drawing or injecting IM |
| 23G | 0.64 mm | Blue | Injecting IM (moderate flow, moderate pain) |
| 25G | 0.51 mm | Orange | Injecting IM or SC (good balance of flow and comfort) |
| 27G | 0.41 mm | Grey | Injecting SC (lowest pain, slowest flow) |
| 28G | 0.36 mm | Clear/Blue (varies) | Insulin syringes for SC |
| 30G | 0.30 mm | Yellow (varies) | Insulin syringes for SC (very slow flow) |
How gauge works: The higher the gauge number, the thinner the needle. A 27G needle is thinner than an 18G needle. Thinner needles cause less pain but make it harder to push thick oil-based medication through.
Choosing the Right Gauge
| Situation | Draw Needle | Inject Needle | Reason |
|---|---|---|---|
| Oestradiol enanthate (EEn) - MCT oil | 20G | 27G | MCT oil is thin; flows easily through small needles |
| Oestradiol valerate (EV) - castor oil | 18-20G | 25-27G | Castor oil is thicker; needs larger draw needle |
| Testosterone cypionate - cottonseed oil | 18-20G | 25-27G | Thick oil; use 27G only if you have patience |
| Testosterone enanthate - sesame oil | 20G | 25-27G | Moderate thickness |
| Any SC injection | 20-22G | 27-30G | SC needs less volume and thinner needles work fine |
Needle Length
| Injection Type | Needle Length | Body Type Considerations |
|---|---|---|
| IM - Ventrogluteal | 1 - 1.5 inches (25-38 mm) | 1.5 inches for larger body types; 1 inch for lean individuals |
| IM - Vastus lateralis (thigh) | 1 - 1.25 inches (25-32 mm) | 1 inch is usually sufficient for thigh |
| IM - Deltoid (arm) | 0.625 - 1 inch (16-25 mm) | 0.625 inches for lean, 1 inch for larger |
| SC - Abdomen | 0.375 - 0.625 inches (10-16 mm) | Standard insulin needle length is typically 0.375 inches |
| SC - Thigh/arm | 0.375 - 0.625 inches (10-16 mm) | Pinch skin fold |
Rule of thumb: For IM, you need a needle long enough to reach through subcutaneous fat into the muscle. A 1-inch needle works for most thigh injections and many ventrogluteal injections. People with higher body fat may need 1.25-1.5 inches for ventrogluteal.
Syringe Reference
| Size | Best For | Markings | Notes |
|---|---|---|---|
| 0.5 mL (insulin) | Very small SC doses (< 0.3 mL) | Every 0.01 mL | Usually fixed-needle insulin syringes |
| 1 mL (tuberculin / slip tip) | Most HRT doses (0.1-1 mL) | Every 0.01 mL | Best for precise dosing |
| 3 mL | Larger doses (> 1 mL) | Every 0.1 mL | Less precise for small doses |
| 5 mL+ | Rarely needed for HRT | Every 0.2 mL | Used only for very large volumes |
Syringe Tip Types
| Type | Description | Best For |
|---|---|---|
| Luer lock | Needle screws on with a twisting motion | Most HRT use - needle stays securely attached |
| Luer slip | Needle pushes on with friction | Quick changes, but needle can pop off under pressure |
| Eccentric tip | Tip is offset to the side | Injecting into small or shallow veins (not commonly used for HRT) |
| Catheter tip | Wider, tapered tip | Irrigating wounds or feeding tubes (not for injections) |
Recommendation: Always use luer lock syringes for HRT injections. The needle stays on securely and will not detach during injection.
Low Dead Space Syringes
Standard syringes leave 0.05-0.1 mL of medication in the needle hub after injection. Over a year of weekly injections, this adds up to 2.6-5.2 mL of wasted medication (roughly 1-2 vials worth).
Low dead space (LDS) syringes reduce this to 0.01-0.03 mL. They have a shorter, narrower hub that pushes more medication through. Many 1 mL insulin syringes are already low dead space by design.
Benefits of LDS syringes:
- Save money by reducing wasted medication
- Reduce tissue trauma (shorter hub means less depth variation)
- Widely available at medical supply stores
Insulin Syringes for HRT
Insulin syringes have a permanently attached needle (fixed-needle design). They come in 0.3 mL, 0.5 mL, and 1 mL sizes with ultra-fine needles (28-31G).
Pros: Very low dead space; thin needles cause less pain; markings are clear and precise; inexpensive.
Cons: Cannot change the needle (you draw and inject with the same needle, which dulls it); only suitable for SC injections; not suitable for thick oil-based medications (too slow to draw and inject).
Verdict: Insulin syringes work well for SC injections of thinner oils (MCT-based EEn) but are not ideal for thick oils (castor oil-based EV) or IM injections.
Sourcing Tips
- Luer lock syringes are preferred (needle screws on securely).
- Buy needles and syringes separately for best flexibility so you can mix and match gauges.
- A 1 mL syringe allows the most precise dosing.
- Draw with 18-20G, inject with 25-27G.
- Buy in bulk to save money -- boxes of 100 needles or 100 syringes are much cheaper per unit than buying individually.
- Filter needles (5-micron) are recommended when drawing from glass ampules to prevent glass shards from entering the syringe.
- Blunt fill needles (18G, no sharp tip) can be used for drawing from vials to avoid coring the rubber stopper.
Needle Phobia and Anxiety Tips
If needles make you anxious, you are not alone. Here are strategies that help:
- Use the smallest needle you can: 27G or 28G for SC injections cause minimal sensation.
- Ice the area: Numbing the skin with an ice cube for 30-60 seconds before cleaning with alcohol can reduce the sensation.
- Distract yourself: Listen to music or a podcast, watch a video, or have a friend talk to you during the injection.
- Breathe: Take a slow deep breath in, breathe out slowly, and insert the needle during the exhale.
- Auto-injectors: Devices like the Union Medico Autoject or similar can hide the needle from view and insert it with a button press.
- Lie down: If you feel faint, lie down for the injection and stay lying down for 5-10 minutes afterward.

Disposal
- Use an FDA-approved sharps container (or a heavy-duty plastic bottle as a temporary alternative).
- Do not put needles in household trash or recycling bins.
- Drop off at pharmacies, hospitals, or designated collection sites.
- Never recap used needles -- this is the most common cause of accidental needlestick injury.
